    Mt. Pleasant Health and Rehab has had a staff member test positive for COVID-19 as of 9/7/2020. This staff member has not worked in the facility since 9/4/2020.
    The employee was tested at our facility on 9/3; was last on shift on 9/4 and we received test results yesterday, 9/7.
    With the new CMS guidelines, we will be testing all residents today and will begin testing employees twice weekly.
    Normal operation of the facility have not been affected.

    As of 11/12/2020 at 5:00pm today, Mt. Pleasant Health & Rehab has an additional 3 residents and 1 employee with Covid-19 positive test results.
    On 11/13/2020 we obtained 3 additional new positive test results for COVID 19. Currently this is all results reported.

    Per guidelines set forth by the Centers for Disease Control, the employee must complete the quarantine requirements and produce either a negative Covid-19 test result or a positive antibody result in order to return to the facility.
    Mt. Pleasant Health and Rehab attempts to follow every guideline and recommendation put forth from the CDC in the event of a National Emergency but may utilize volunteers to ensure the safety of our residents.

    11/18/2020 at 12:15pm, Mt. Pleasant Health and Rehab has had 4 more residents test positive for COVID-19. To attempt to limit the COVID-19 virus the facility is attempting to follow CDC Guidelines as closely as possible and monitor any and all changes. Any resident that tests positive is being relocated in a separate zone which has been designated and quarantined for only COVID-19 residents. Mt. Pleasant Health and Rehab attempted to go as long as we could without a case to keep your loved one safe and we will continue to do the exact same after the fact. Please keep the same support and faith with us as we go through these trying times. Thank You.

    On February 24th, 2021 the Tennessee Department of Health announced that visitation restrictions will be lifted at the state level as of February 28th, 2021. What this means for Long Term Care Facilities in Tennessee is that facilities will still be required to take guidance and follow visitation guidance from Federal Departments such as The Centers of Medicare and Medicaid Services and The Centers for Disease Control and Prevention. This does not mean that facilities will be opening their doors for visitation freely. Visitation will be still considered by each facility on a case by case basis depending on such factors like specific county positivity rates. Meaning if a county is still considered having a high positivity rate (10% or more), then visitation may not be a current option for the facilities residing in that county. Mt Pleasant Health and Rehab is monitoring Maury Counties positivity rates on a daily basis to try to accommodate visitations but will always keep safety as our top priority. We appreciate the understanding and patience from the residents, families and staff.

    Mt. Pleasant Health and Rehab has began visitations in the front entry between family members and their loved one with provisions. Residents that have been fully vaccinated can now have actual contact with their family members. Fully vaccinated is described below.

    *Fully vaccinated refers to a person who is ≥2 weeks following receipt of the second dose in a 2-
    dose series, or ≥2 weeks following receipt of one dose of a single-dose vaccine, per the CDC’s
    Public Health Recommendations for Vaccinated Persons.

    As of 7/30/21 at 7:06PM Mt Pleasant Health and Rehab has had 1 staff member test positive for COVID-19, last day worked was 7/30/21. All residents were tested and are negative.     As of 8/9/21 at 3:11pm, we have had one additional staff member who tested positive on 8/3/21 at hospital, last day worked was 7/25/21. All residents remain negative. Twice a week testing for all residents and staff will continue for two weeks after last staff positive. Twice a week testing for unvaccinated staff will continue per state and CDC guidelines in regards to Maury County, TN COVID positivity rate. Visitation will continue as per usual, per appointment.     As of 9/8/21 at 6:48am, we had one staff member test positive for COVID while at home. The last day worked was 9/3/21 and at that time the employee was negative. The employee has remained off work since 9/3/21.

    As of 10/7/2021 Mt Pleasant Health and Rehab has had 1 resident test positive at 4:02pm. The remaining staff and residents are being tested and currently all remaining staff and residents tested negative. Testing will continue twice a week until outbreak is resolved. Visitation will continue by appointment in the front lobby foyer but it is recommended to visit outside or by window if possible. Thank You.

    To Our Residents, Family and Staff:

    The Centers for Medicare and Medicaid Services (CMS) released an updated memorandum on Visitation in Nursing Homes; this is exciting news, as we are no longer required to restrict visitation in our facility! We are required to make sure that you are educated on and understand the risks associated with visiting in a congregate care setting and with the practices that we will all have to follow in order to reduce the risk of infection within our facility. This information is detailed below; please review the information carefully and let us know if you have any questions.

    Ways you can help reduce the spread of infections:

    • Visitors who have a positive viral test for COVID-19, symptoms of COVID-19, or currently meet the criteria for quarantine, should not enter the facility.
    • Hand hygiene (use of alcohol-based hand rub is preferred)
    • Face covering or mask (covering mouth and nose)
    • Physical distancing at least six feet between people, in accordance with CDC guidance
    • Appropriate use of Personal Protective Equipment (PPE)

    Outdoor visits are preferred but all above steps should still be followed.

    Indoor visitation – in addition to all of the above ways to reduce the spread of infections:
    • Limit your visit to only the person you came to see
    • If there is a roommate, pull the curtain between the beds
    • Perform hand hygiene frequently throughout the visit

    While not recommended, residents who are in quarantine or isolation can receive visitors. Be aware that there are increased risks of spreading Covid-19 or other infections when you visit someone in quarantine or isolation. Additional safety precautions will be required to reduce the risk as much as possible, including wearing a gown, gloves and a face shield.

    Visiting during an outbreak investigation is discouraged. If you still chose to visit, be aware that the potential risk to both you and your loved one you are visiting is higher.

    Did you know that the CDC reports the community transmission for all counties in the country? Many people do not even know they have the infection, but they are still able to spread Covid-19. When our county is in a moderate to high level of community to transmission, we are encouraging everyone to get tested prior to visiting in an effort to prevent someone unknowingly bringing Covid-19 into our resident’s home. We are able to offer testing to visitors during specific times. Please call ahead to schedule a time to be tested. Testing is quick and results take 15 minutes. If not able to get tested at our facility, please get tested in the 2-3 days before coming to the facility. The website to visit to find the CDC Community Transmission rate is https://covid.cdc.gov/covid-data-tracker/#county-view

    Resident outings also have specific considerations. Please remember that while not in the facility, the resident and everyone around them must continue to follow all recommended infection prevention practices including wearing a face mask, physical distancing, and hand hygiene.

    Upon return, we are required to take the following action:
    • Screen residents upon return for signs or symptoms of Covid-19
    • If possible close contact to an individual with Covid-19 while outside of the nursing home occurred, we must test the resident and follow CDC guidelines
    • If gone for 24 hours or longer, we must quarantine all residents who are not fully vaccinated.

    We are excited to welcome you back for in-person visiting! We are still able to assist with televisits, and we are also still able to schedule a visit to ensure you have a designated private visiting space reserved. Thank you for your continued patience as we all adapt to these updates.

    Currently, we have 2 new cases of Covid-19.
    One staff member developed symptoms on 1/10/22, was tested on 1/11/22 and received positive result from test today, 1/13/22.
    One resident tested positive today 1/13/22 during our twice weekly testing.
    We will continue to perform our twice weekly testing on all residents and staff for the next 2 weeks, regardless of vaccination status.

    As of 7/5/22 at 12am we have had 2 additional staff members test positive for COVID. This makes a total of 10 residents and 4 staff members with COVID currently. We are currently in crisis staffing status and will be following CDC and CMS guidance

    As of 10/17/22 at 2pm, we have had 1 staff member test positive for COVID-19. Contact tracing has been performed and we believe that no residents have been exposed at this time. Close contacts and members of department effected will test per CDC guidelines.
